RE:      IRS Form 1095

One of the requirements of the Affordable Care Act (ACA) is that individuals must be able to verify their health insurance coverage.  The IRS Form 1095 provides information about the health care coverage that you had or were offered during the previous year.

·         The IRS has advised taxpayers that they do NOT need to delay filing their tax returns until they receive Forms 1095-B and/or 1095-C.  
·         You are NOT required to submit a copy of the 1095 form when you file, you may use other information about your health coverage to prepare your returns.
·         Employees may receive one or more of the IRS Form 1095.  UC has engaged Unify HR to produce and mail the Form 1095 to applicable employees.

o   Form 1095-B, - Mailed to the individual by the Health insurance provider or Unify HR, depending on the plan you were enrolled in. Includes information about who was covered and when. 

o   The 1095-C, Employees who are considered full-time by ACA standards (those who work 75 percent time or more) will receive this form. Form is now available on At Your Service Online (AYSO) and paper copies will be mailed by Unify HR by March 2, 2017 to anyone who did not elect electronic delivery. Dependents will only be listed on the 1095-C form if you were enrolled in UCCare or Core.
