RE:        Redeem Wellness Incentive Awards by 1/31/16

This message is for participants in the Optum Wellness program who earned 7,500 points by the December 15, 2015 deadline and still need to redeem the $75 incentive reward on the Optum website.

Deadline to redeem the award is January 31, 2016.

To redeem your incentive award:

Go to https://client.myoptumhealth.com/uclivingwell
Log in to your account; go to I Get
Continue even though the page states “This rewards program ended”
Select “click here”
From the left-hand side, click My Rewards to open the menu
Select Redeem Rewards
Click on Visa® Reward Card
Enter 7500 in the amount field
Select Add to Cart and complete your order 
Await delivery of your reward card
Please allow a minimum of 20 days for processing and shipping to receive your Visa® Reward Card. Please make sure your delivery information is correct.
